For and in consideration of my participation in the League Joe EVENT (hereinafter the "EVENT"), I hereby, for myself, my heirs, executors, successors, administrators and assigns assume any and all risks associated with my participation in the EVENT, and release and waive any and all rights and claims that I may now, or in the future, have against League Joe, its agents, employees, directors, officers and affiliates from any and all claims from injuries, damage, wrongful death or loss, including, but not limited to, any alleged negligence, which I may have or which may accrue to me from my participation in the EVENT.

I understand and agree that League Joe, its agents, employees, directors, officers and affiliates are not responsible for any injury or property damage arising out of my participation in the EVENT, even if caused by their negligence. I understand that participation in the EVENT involves certain risks, including, but not limited to, serious injury and death. I am voluntarily participating in the EVENT with knowledge of the danger involved and agree to accept all risks of participation.

I further agree to indemnify and hold harmless and defend League Joe, its agents, employees, directors, officers, and affiliates from any and all claims resulting from injuries, damages and losses sustained by me or arising out of, connected with or in any way associated with the activities of the EVENT or my participation in the EVENT and related activities. I acknowledge that League Joe, is not responsible for any inclement weather which may affect the EVENT and release and waive all claims against League Joe, its agents, employees, directors, officers, and affiliates for damage and loss that may arise there from. I understand that this document is intended to be as broad and inclusive as permitted by the laws of the State of IN and agree that if any portion of this Agreement is invalid, the remainder will continue in full legal force and effect.

I further agree that any legal proceedings related to this waiver will take place in Indianapolis, IN. If participant is under 18, participant's parent or legal guardian must read and sign this form and understand that he or she is giving up legal rights and remedies on behalf of the participant and his/her family estate, heirs, and/or assigns.
